The items of equipment listed in Section 4 will be included on my inventory in line with the national terms of connection contained within the Distribution Connection and Use of System Agreement.

Please provide the details of the Lighting Authority or other organisation accepting responsibility for the energy charges and which electricity Supplier they wish to nominate.

Please complete the form, following the below steps, which should take about 20 minutes to complete.
